What are the two medicines?

Amoxicillin is a penicillin-type antibiotic. It kills susceptible bacteria by interfering with bacterial cell-wall formation.

Clavulanate is a β-lactamase inhibitor. Some bacteria produce β-lactamase enzymes that can destroy amoxicillin. Clavulanate inhibits many of these enzymes, allowing amoxicillin to work against additional susceptible bacteria.

So this is a combination antibiotic, not a probiotic or vitamin.

What infections can it be used for?

Amoxicillin/clavulanate is commonly prescribed for susceptible bacterial infections such as:

  • Ear infections (otitis media)
  • Sinusitis
  • Certain throat/respiratory infections
  • Pneumonia
  • Some skin and soft-tissue infections
  • Certain urinary-tract infections
  • Some dental infections

The exact indication depends on the child's symptoms, examination and, when appropriate, culture results.

The important part: “DS”

DS = Double Strength.

However, don't determine the child's dose from “DS” alone. Different manufacturers can have different concentrations.

The photograph clearly shows “5 gm/30 mL” on the front, but the small print that specifies the amount of amoxicillin and clavulanate per 5 mL isn't sufficiently readable in this image. That concentration is essential for calculating the correct dose.

For example, commonly encountered pediatric formulations contain 400 mg amoxicillin + 57 mg clavulanate per 5 mL, but I would not assume that is the composition of this particular bottle without reading its label.

Why the child's weight matters

Pediatric amoxicillin/clavulanate dosing is generally calculated according to the amoxicillin component in mg/kg/day, with the dosing schedule depending on the infection and formulation.

Therefore, the same “DS” bottle can require different volumes for children of different weights.

Don't give a particular number of mL based only on this photograph.

Common side effects

The most common problems are:

  • Diarrhea/loose stools
  • Nausea or vomiting
  • Abdominal discomfort
  • Rash

Taking the medicine at the start of a meal can improve gastrointestinal tolerance for many amoxicillin/clavulanate formulations.

Important allergy warning

Because amoxicillin is a penicillin antibiotic, it can cause allergic reactions.

Seek urgent medical attention for:

  • Difficulty breathing
  • Swelling of the lips, tongue or face
  • Widespread hives
  • Severe skin blistering/peeling

A history of a serious reaction to penicillin or certain cephalosporins should be reported to the prescribing clinician.

After mixing the dry syrup

This bottle contains powder that needs to be reconstituted. The correct amount of water and storage instructions should come from the product's instructions/pharmacist.

Don't simply fill the bottle completely with water unless the label specifically instructs that.

After reconstitution, shake well before each dose and use an appropriate oral syringe/measuring device.

How this relates to the medicines you've shown

You have now shown several medicines:

MedicineMain role
Cefcum-S 375Ceftriaxone + sulbactam — injectable antibiotic
AMCA-500Amikacin — injectable antibiotic
ACZID-250Ceftazidime — injectable antibiotic
ACCTRONOndansetron — anti-vomiting medicine
Acclone-50Nandrolone decanoate — anabolic/androgenic steroid
PRECOT PlusPrebiotic + probiotics + zinc
MOXYCOT-CV DSAmoxicillin + clavulanate — oral antibiotic

There is therefore substantial antibiotic overlap among the medicines you've shown. These drugs should not be combined, substituted, or continued simply because they are all antibiotics. If these medicines belong to the same child/patient, the exact prescription and diagnosis are important.

One thing I would particularly want to verify: the image says “for paediatric use only.” If you tell me the child's age and weight and upload a clear photo of the back/side of the MOXYCOT-CV DS box, I can identify the exact mg of amoxicillin + clavulanate per 5 mL and explain what the prescribed dose means.
