The image shows DICLOTT-SP Tablets by Accott. The front of the carton clearly identifies the three active ingredients:
Diclofenac Potassium + Paracetamol + Serratiopeptidase
This is a prescription pain-relieving and anti-inflammatory combination, generally used for painful conditions accompanied by inflammation and swelling.
The exact strengths are not printed clearly enough on the front panel in this photograph, so I don't want to guess the quantities. The formulation contains:
The pack contains 10 × 10 tablets = 100 tablets.
2. Diclofenac PotassiumDiclofenac potassium is a non-steroidal anti-inflammatory drug (NSAID).
It works primarily by inhibiting the enzymes:
COX-1 and COX-2 (cyclooxygenase)
These enzymes participate in the production of prostaglandins, which contribute to:
pain
inflammation
swelling
fever
Simplified:
Diclofenac → COX inhibition → ↓ prostaglandins → ↓ pain + ↓ inflammation
Diclofenac can provide relief in conditions such as:
musculoskeletal pain
arthritis-related pain
dental pain
acute injuries
postoperative pain
painful inflammatory conditions
Diclofenac is available in different salt forms. Diclofenac potassium is a particular salt formulation commonly used in relatively rapidly acting oral preparations.
3. ParacetamolParacetamol is an analgesic (pain reliever) and antipyretic (fever reducer).
It is not considered a conventional NSAID and has considerably less peripheral anti-inflammatory activity than diclofenac.
Its principal clinical roles are:
Pain reduction + fever reduction
Combining paracetamol with diclofenac can provide additional analgesic effect through different mechanisms.
Paracetamol is generally well tolerated at recommended doses, but excessive total daily intake can cause serious liver injury.
This is particularly important when someone is already taking another medicine containing paracetamol.
Do not add another paracetamol-containing painkiller without checking the total daily dose.
4. SerratiopeptidaseSerratiopeptidase (also called serrapeptase) is a proteolytic enzyme.
It has been marketed and studied for its potential effects on inflammatory swelling and tissue edema.
The proposed rationale is:
Serratiopeptidase → proteolytic activity → proposed reduction in inflammatory edema → ↓ swelling
It is often included in combination tablets intended for painful, swollen conditions.
Unlike diclofenac and paracetamol, serratiopeptidase is not a conventional analgesic.
Evidence supporting serratiopeptidase for routine pain/swelling is less established and more variable than the evidence for standard analgesics/NSAIDs. Therefore, the pain-relieving effect of this combination should not be attributed solely to serratiopeptidase.
5. Why are all three combined? The formulation is designed to address different components of painful inflammation:
↓ inflammation + ↓ pain
↓ pain + ↓ fever
intended to help with inflammatory swelling

Relief of painful,
inflammatory conditions6. Common usesA doctor may prescribe this type of combination for short-term treatment of moderate pain associated with inflammation, for example:
acute musculoskeletal pain
sprains and strains
joint inflammation
arthritis-related pain
dental pain
postoperative pain
soft-tissue inflammation
painful swelling following injury
The exact indication depends on the patient's condition.
7. Important side effectsBecause this combination contains diclofenac, the most important precautions relate to NSAID toxicity.
Diclofenac can cause:
acidity
indigestion
stomach pain
nausea
gastritis
stomach ulcer
gastrointestinal bleeding
The risk is higher with prolonged use, higher doses, older age, previous ulcer/bleeding, alcohol use, or concomitant medicines that increase bleeding risk.
NSAIDs can reduce kidney blood flow in susceptible individuals and may cause:
fluid retention
worsening kidney function
increased blood pressure
Diclofenac can increase cardiovascular risk, particularly with higher doses and prolonged treatment.
Both diclofenac and paracetamol are metabolized by the liver. Liver injury is uncommon at appropriate doses but becomes particularly important with excessive paracetamol intake or certain risk factors.
8. Who should be particularly cautious?Medical advice is particularly important if the patient has:
stomach ulcer or previous GI bleeding
kidney disease
liver disease
heart disease
uncontrolled hypertension
asthma triggered by aspirin/NSAIDs
bleeding disorders
dehydration
significant alcohol use
It is also important to tell the doctor about other medicines being taken.
9. Important drug interactionsBecause of the diclofenac component, caution is required with medicines such as:
other NSAIDs — ibuprofen, naproxen, aspirin used for pain, etc.
anticoagulants such as warfarin
antiplatelet medicines
corticosteroids
some antidepressants that increase bleeding risk
certain blood-pressure medicines
diuretics
some medicines affecting kidney function
Taking diclofenac + ibuprofen + naproxen, for example, generally increases toxicity without providing a proportionate benefit.
10. Do not duplicate paracetamolThis is one of the most important practical points.
If DICLOTT-SP contains paracetamol, avoid accidentally taking another product containing:
paracetamol
acetaminophen
PCM
at the same time unless your doctor has specifically advised it.
Many cold/flu preparations also contain paracetamol.
11. Is it a steroid?No.
DICLOTT-SP does not contain a corticosteroid such as:
prednisolone
methylprednisolone
dexamethasone
Diclofenac is an NSAID, not a steroid.
12. Is it an antibiotic?No.
None of the three listed ingredients is an antibiotic.
It will not treat a bacterial infection.
13. Is it suitable for long-term daily use?Generally, a combination containing diclofenac should not be used continuously for long periods unless specifically prescribed and monitored.
The goal is usually to use the lowest effective dose for the shortest appropriate duration, because NSAID-related gastrointestinal, renal and cardiovascular risks increase with exposure.
Active ingredients:
Diclofenac potassium → NSAID; analgesic + anti-inflammatory
Paracetamol → analgesic + antipyretic
Serratiopeptidase → proteolytic enzyme marketed for inflammatory swelling
Pain + inflammation + associated swelling
Stomach/intestinal bleeding, kidney problems, cardiovascular risk, liver toxicity and accidental duplication of paracetamol are the major issues to keep in mind.
One important limitation of this photograph: the front of the box does not give me a readable strength for each ingredient. If you upload a clear photograph of the back/side composition panel or the blister strip, I can give you the exact mg of diclofenac potassium, paracetamol and serratiopeptidase per tablet, and explain each quantity in detail.
