Clobetasol Propionate, Neomycin & Miconazole Nitrate Cream

1. What the three ingredients do

So Derma-AC combines an antifungal + antibiotic + powerful steroid.

2. The important part: clobetasol

Clobetasol is the ingredient that requires the most caution. It is a highly potent topical corticosteroid. Prolonged or inappropriate use can cause:

• thinning of the skin
• stretch marks
• visible small blood vessels
• changes in pigmentation
• folliculitis/acne-like eruptions
• secondary infection
• irritation or burning

Clobetasol can also suppress inflammation so effectively that an infection may appear to improve temporarily while the underlying infection persists. Official prescribing information specifically advises appropriate antimicrobial treatment when infection is present and discontinuing clobetasol if the infection isn't responding appropriately. 3. Why Derma-AC is not simply an antifungal cream

If someone has ordinary ringworm, for example, there are two separate effects here:

Miconazole → attacks the fungus
Clobetasol → suppresses the body's inflammatory reaction

The second effect can make itching and redness disappear quickly, but it doesn't kill the fungus.

For uncomplicated fungal infections, dermatology guidance generally recommends an antifungal alone rather than an antifungal/high-potency steroid combination. AAFP

4. Neomycin is another consideration

Neomycin can cause allergic contact dermatitis in some people. If a rash becomes more irritated, develops new redness/itching, or fails to heal while using the cream, that possibility should be considered rather than simply applying more cream.

5. Be particularly careful about where it is applied

Clobetasol-containing preparations deserve extra caution on thin or sensitive skin, particularly the:

• face
• groin/genitals
• armpits
• skin folds

It should also not be put in the eyes, mouth, or inside the vagina unless a clinician has specifically prescribed an appropriate formulation for that purpose.

Comparing all four creams you've shown

You have now shown:

Lulek is the outlier: it is essentially a dedicated topical antifungal. The other three are multi-ingredient preparations that contain clobetasol.

Because of that, I would not automatically use these four creams interchangeably or apply several of them together to the same rash. Using more than one steroid-containing product can increase total corticosteroid exposure. DailyMed
