What clindamycin does

Clindamycin is a lincosamide antibiotic. It works against susceptible bacteria, particularly certain Gram-positive bacteria and anaerobic bacteria, by interfering with bacterial protein production.

It may be used for serious infections involving, for example:

  • Skin and soft tissues
  • Lungs/respiratory tract, including certain bacterial pneumonias
  • Abdomen, such as some intra-abdominal infections
  • Bones and joints
  • Pelvic/gynecological infections
  • Certain bloodstream infections

The exact use depends on the suspected or confirmed bacteria and its antibiotic susceptibility. DailyMed

How it is given

The image specifically says I.M./I.V. use only.

For IV administration, it must be diluted and administered as an infusion rather than injected rapidly as an undiluted IV bolus. Product information specifies that IV infusion should generally take at least 10–60 minutes, depending on the dose and preparation. Medicines.org.uk

So this is not an injection to administer yourself.

The most important warning: diarrhea

Clindamycin has a particularly important association with antibiotic-associated diarrhea and Clostridioides difficile (C. difficile) colitis. This can sometimes be severe and can occur during treatment or even weeks afterward. DailyMed

Seek medical attention if someone receiving clindamycin develops:

  • Severe or persistent watery diarrhea
  • Blood or mucus in stool
  • Significant abdominal cramps/pain
  • Fever with diarrhea

Do not simply treat significant antibiotic-associated diarrhea with an anti-diarrheal medicine without medical advice.

Other possible adverse effects

Possible effects include:

  • Nausea or vomiting
  • Abdominal pain
  • Diarrhea
  • Skin rash/itching
  • Injection-site pain or irritation
  • Changes in liver-function tests

Rare but serious allergic and skin reactions—including anaphylaxis, Stevens–Johnson syndrome, toxic epidermal necrolysis and DRESS—have been reported. Medicines.org.uk

Important interactions/precautions

The healthcare professional should know if the patient:

  • Has previously reacted to clindamycin or lincomycin
  • Has a history of colitis or significant gastrointestinal disease
  • Takes warfarin or another vitamin-K antagonist, because coagulation tests may need monitoring
  • Is receiving certain neuromuscular-blocking medicines, because clindamycin can enhance their effects. Medicines.org.uk

So Clindacot-600 is the antibiotic among the four.

If all four injections were prescribed together for the same patient, I can also explain the likely clinical purpose of giving these four together, which symptoms each targets, and which combinations require particular caution.
