1. What is it used for?

Both medicines are antiparasitic drugs, but they act against different parasites.

Albendazole is a broad-spectrum deworming medicine. It is used against several intestinal worms, including roundworms, hookworms and certain other helminths.

Ivermectin is active against particular parasitic worms and some ectoparasites. The combination of albendazole 400 mg + ivermectin 6 mg has been approved in India for treatment of intestinal helminth infections and suppression of microfilariae, particularly in Wuchereria bancrofti infection. CDSCO

So this is not a general-purpose antibiotic and isn't intended for ordinary bacterial infections.

2. How the two medicines work

Albendazole

• Interferes with the parasite's microtubules and glucose utilization.
• This eventually immobilizes and kills susceptible worms.

Ivermectin

• Acts on chloride channels in susceptible parasites.
• This causes paralysis of the parasite and its eventual death.

Using the two together can provide broader antiparasitic activity than either drug alone in situations where the combination is specifically indicated.

3. Important: the 400 mg + 6 mg strength does NOT mean everyone should take one tablet

The correct dose and number of doses depend on the particular parasite, age/weight, diagnosis and medical circumstances. I would not recommend taking this particular combination simply because you suspect you have "worms."

For example, ivermectin dosing for some infections is weight-based, while albendazole regimens vary considerably according to the infection.

4. Common side effects

Possible effects include:

• Nausea
• Abdominal pain/discomfort
• Diarrhea
• Vomiting
• Headache
• Dizziness
• Weakness/fatigue
• Rash or itching

Ivermectin can also cause dizziness, drowsiness and, less commonly, more serious neurological effects. DailyMed

Albendazole can cause gastrointestinal symptoms and elevations in liver enzymes. DailyMed

5. Important precautions

Pregnancy is particularly important. Albendazole has potential embryo-fetal toxicity and should generally be avoided during pregnancy unless a clinician determines that treatment is necessary. Ivermectin also should not be used during pregnancy because adequate safety has not been established. DailyMed

Tell the doctor if you have:

• Liver disease
• Abnormal liver-function tests
• Previous allergy to albendazole/ivermectin or related medicines
• Pregnancy or possibility of pregnancy
• A child who is going to receive the medicine
• Other medicines being taken

Longer or repeated courses of albendazole can require liver-function and blood-count monitoring, because significant liver injury and bone-marrow suppression are recognized risks. DailyMed

6. A particularly important distinction

This tablet should not automatically be used for scabies, COVID-19, viral infections, or nonspecific itching.

Ivermectin does have approved uses for certain parasitic conditions, but the appropriate dose and duration depend on the diagnosis. The fact that ivermectin is present in this tablet does not mean that every condition for which ivermectin has been discussed should be treated with this particular 6-mg combination tablet.
